Procedure to book your Consultation

To book a consultation share following details of yours to opd coordinators’s WhatsApp or email to nephrology.ckdr@gmail.com

With following documents:

1. Previous prescription of Dr Bhatt or other doctors
2. Latest investigations
3. Your main complaints in your own words
4. Make and share your list of medicines, doses, timings you are taking currently preferably a typed one

Following suggestions will make your opd visit convenient:

1. Arrange your medical documents into two files, one for prescriptions and another for investigations and arrange documents date wise.
2. Bring your medicine box while you visit to clinic
3. if you are a diabetic or BP patient please make your Sugar monitoring chart and BP monitoring chart to discuss with Dr Bhatt.
4. List your points for discussion in clinic for better understanding
5. You are allowed record Dr Bhatt’s Voice or Video for understanding prescription with his prior permission.
6. If you need special assistance like wheelchair ect please let me know. Home clinic has two steps to climb.

Fee : Each visit is payable. If patient is unable to pay he or she can request Dr. Bhatt. * Poor patients are seen free. If you are referring any poor patient, please inform me so that I can provide due attention.

If you are not able to find or understand any medicine please inform us first. I can help you arranging your medicines and labs if needed.

⁠In OPD discuss your case but Don’t criticise your previous doctors, everyone tries to do their best for patients.
